What's Actually Happening to Your Hair

Every hair on your head cycles through three phases: anagen (active growth, lasting years), a brief transition, and telogen (a resting phase that ends when the hair sheds to make room for the next one). At any given time, roughly 85–90% of your scalp hair is quietly growing in anagen — that's why normal daily shedding feels unremarkable.

Pregnancy changes the math. Elevated estrogen holds more follicles in the growth phase than usual, which is the biological reason so many women describe their hair as thicker and fuller than ever during pregnancy — it isn't in your head, and it isn't extra hair, it's held hair.

Then you give birth, and estrogen drops sharply. All those follicles that were held in growth get released into the resting phase at the same time. About three months later, they shed together — not gradually, one strand at a time, but suddenly, in the handfuls that make people panic in the shower. The clinical name for this is telogen effluvium: a diffuse, temporary, non-scarring shedding event. It is not a disease, and — this matters — it is not the same thing as ongoing pattern thinning.

This mechanism is foundational dermatology, first documented in a landmark 1960 study and confirmed repeatedly since — it's the reason nearly every major hospital and dermatology body describes postpartum shedding the same way.

The Part Most Guides Skip

You've probably read that postpartum hair loss is caused by a "DHT surge." We looked hard for the study behind that sentence. We couldn't find one. The best available endocrine research tracking hormones from pregnancy through postpartum shows testosterone rising slightly in the second and third trimester, then gradually falling after birth — the opposite direction of a surge, and it doesn't isolate DHT specifically at all.

Here's what the research does show, and it's more interesting than a single villain hormone: a 2024 study of 200 postpartum women found that while shedding-only telogen effluvium was actually the minority presentation, 56% of women had telogen effluvium happening alongside an underlying androgenetic (pattern-thinning) component at the same time.

The likely explanation isn't a new DHT surge — it's that pregnancy's estrogen was masking a pattern-thinning tendency some women already carry, the way it masks the shed. Once estrogen drops, both become visible together. That's a real, clinically documented phenomenon. It's just a different, more precise one than the marketing shorthand.

Why this distinction actually matters to you

If your product only addresses the estrogen-driven shed, it may miss the androgen-sensitive piece that a real subset of postpartum women are also dealing with. If it only addresses DHT, it misses the acute shed entirely. It's a reasonable case for addressing both angles at once — not because of a "surge," but because both mechanisms genuinely coexist for a lot of women in this window.

Why Generic Hair Products Weren't Built for This Window

Most hair-loss products on the market were formulated for a broad population — aging, menopause, general pattern thinning — not specifically cleared or considered for pregnancy and breastfeeding. That's not a marketing talking point; it shows up directly in the safety data:

Topical minoxidil
Case-report data documents infant hypertrichosis (excess hair growth) from skin contact during breastfeeding. Lactation database rates evidence as insufficient to confirm safety.
Finasteride

Hormones aren't the only postpartum factor that affects hair. Before assuming it's "just" telogen effluvium, it's worth knowing what else commonly plays a role:

Thyroid

Postpartum thyroiditis affects an estimated 5–10% of postpartum women, typically surfacing 4–8 months after birth. Its hypothyroid phase lists hair loss as a recognized symptom — which is exactly why shedding that lingers past the usual window deserves a TSH and free T4 check, not just patience.

Iron (ferritin, specifically)

A standard blood count can look normal while stored iron (ferritin) is still low — and low ferritin is independently linked to hair shedding. Ask for ferritin by name, not just a CBC.

Vitamin D & zinc

Both are frequently checked in hair-loss workups, and vitamin D deficiency shows up often enough in shedding patients to be worth testing — though researchers are candid that the evidence on causation, not just correlation, is still mixed.

A practical note on biotin

If you're taking a high-dose biotin supplement and your doctor orders thyroid labs, mention it. It can distort the results.

Ordinary telogen effluvium is diffuse, gradual to notice week-to-week, and self-limiting. A few signs suggest it's worth a closer look sooner rather than later:

Shedding hasn't started slowing by 6 months. That's the point most sources expect improvement to begin.

It hasn't resolved by 12 months. Beyond a year, it's reasonable to rule out thyroid, iron, or other contributing causes rather than keep waiting.
